A Note to parents and students about the Forms for Youth activities:

Form A (The YOUTH Liability Waiver) is good for a full year (June 1 - May 31). If your insurance information, address, or phone number changes some time during the year please fill out a new Form A. All youth MUST have a Form A on file to attend Youth Ministry Activities.

Form B is the permission slip/consent for emergency treatment for individual activities. All youth MUST have a Form B for each event they attend.

Form D (The ADULT Liability Waiver) is also good for a full year (June 1- May 31) All adults MUST have a Form C on file to attend Youth Ministry Activities.

as well as to keep pace with the policies of the Catholic Diocese of Ft. Worth.

Who can go to what??
7th - 8th grade activities are open to all incoming 7th graders through graduating 8th graders.
9th - 12th grade activities are open to incoming 9th graders through graduating seniors.
7th - 8th grade activities are open to all incoming 7th graders through graduating seniors.

July10th - Mystery Trip #2
What is this? Mystery trips are for 7th-12th graders. Throughout the summer St. Bart’s youth group will go someplace “strangely fun.” Meaning the place will be strange and fun… duh! All you do is show up at church at the appointed time, the rest is a mystery. Day trips will cost $20.00 or less. Parents who want to know ahead of time where we are going may call Jeff or Eric. But please keep the secret!

Camp Ft. Worth Session – Two sessions
July 15-20 in Wichita Falls ~ August 5-10 in Fort Worth
This is a week long service camp. Each day we will experience a different work site (examples are Salvation Army, Food Bank and others). We will stay the week at Notre Dame High School in Wichita Falls and serve in the agencies that are in that community. Each evening we will have free time, small group time, and prayer time. There will be 100 other youth from around the diocese there. I can take only 10 youth from St. Bart's so sign up NOW to attend this year’s camp. It will be an awesome experience.

July 31 Six Flags - All 7th - 12th graders:
The office has Six Flags tickets for $25.00. Meet at 9:00 AM return at 9:00 PM

Softball: All 7th - 12th graders.
There will be practice at the church most Sundays from 5:00 - 8:00 PM starting in August. We practice in preparation for a one day tournament September 22nd with the other Catholic Churches in the diocese.
